Every user of the sk8 factory skatepark has to complete the waiver form below on/or prior to the first entry, print it off and get it signed.

This must be completed in full and signed by a parent or guardian over 18, If it is not you will be unable to use the skatepark.

Users under 10 years must have the parent or guardian present whislt using the skatepark, 10 years and above can be left under our supervision.
